Statement of Ultimate Consignee

As a member of The MathWorks Book Program, you may be eligible for complimentary access to relevant products for development of this book project. Authors are required to complete the following certification before we can provide software for your project.

I/We shall promptly send a supplemental statement to The MathWorks disclosing any change of facts or intentions set forth in this statement which occurs after this application is submitted.

I/We will not re-export, resell, or otherwise dispose of any items related to this statement contrary to the U.S. Export Administration Regulations except as specifically authorized by the U.S. Export Administration Regulations (15 CFR parts 730-114), or by prior written approval of the Bureau of Industry Security and/or The MathWorks.
